Gambaran Potensi Medication Error Pada Proses Compounding Dan Dispensing Di Apotek Kabupaten Bone Bolango Medication error are incidents that cause harm to patients due to the use of medication during healthcare treatment, which are, in fact, preventable. These errors can occur at any stage of the medication process, including prescribing, transcribing, compounding, dispensing, and administration. Medication errors remain a significant health issue, potentially leading to various adverse outcomes for patients, ranging from mild risks to the most severe consequence, death. This study aims to identify the potential for medication errors during the compounding and dispensing processes in pharmacies located in Bone Bolango Regency. The study employed a descriptive qualitative design with non-experimental approach. The study was conducted over a period of two months and involved 13 respondents, consisting of pharmacists and pharmacy assistants. Data were collected using a checklist and analyzed using data processing software. The results indicated the presence of potential medication errors in the compounding process based on the parameters and in the dispensing process based on 5 parameters, with the highest percentage observed being 57.1% and 30.7%, respectively.
